Minimum age
The minimum age for a CDL is 18. Under 21, the license is an intrastate CDL valid only in Arizona, and the driver may not transport hazardous materials.
CLP fee
A Class A or B permit with no endorsements is $25. A Class C permit, which must have a passenger endorsement, is $12.50. A hazardous materials endorsement is not allowed on a Class C permit. A duplicate permit is $2.
CDL fee
For a new license or a transfer from another state, Class A or B is $25. Class C, which must include a passenger endorsement or a hazardous materials endorsement, is $12.50.
Skills-test fee
A required road or skills test in a Class A or B vehicle is $25, and in a Class C vehicle is $12.50. In the passenger and school-bus section, the page lists $25 for a new applicant and $5 for an existing CDL holder. Adding a passenger or school-bus endorsement to an existing CDL is $5.
ELDT
FMCSA requires entry-level driver training from a certified provider before the skills test for a first Class A or Class B CDL, a B-to-A upgrade, or a first school bus, passenger, or hazardous materials endorsement. The page says the training covers theory, range, and road. Hold the permit at least 14 days before the skills test.
Medical card
The physical must be done by an examiner on the National Registry. That examiner submits the medical examiner certificate electronically. Allow 24 hours before an office visit. A driver restricted to Arizona, the K restriction, still submits the paper certificate and long form. The certificate is renewed every 24 months, or sooner if the examiner or a rule requires it.
